Do you have a child with a birthday coming up?

Submit his or her photo for the News 12 This Morning Birthday Club!

Note: The parent or guardian of the child must be the one to send in the information and photo.

To participate, email the following information to birthdays@wrdw.com:
1) The exact date of the birthday (Birthday Club participant must be 16 years old or younger)
2) The birthday person's full name, mailing address and phone number

The following are optional:
3) A photo of the birthday person (Note: These have to be emailed in)
4) The birthday person's age

All entries must be received no later than two weeks before the birthday!
