2) The birthday person's full name, mailing address, and phone number

The following are optional:

3) A photo of the birthday person
4) The birthday person's age

All entries, including emailed entries, must be received no later than two weeks before the birthday!

We're sorry, but we can no longer send birthday photos back. Please send us pictures you don't mind us keeping.
You can also email them.

Each weekday, one lucky birthday person will be selected to receive a free birthday cake from the Cake Factory! The winner will receive a gift certificate in the mail in time for his or her birthday.
